OCTO Ops has two printable, one-off document types: an MT Delivery slip and an Empty Removal permit. Each is created, then printed — there's no list or profile to come back to afterward.
Before you start
Forms is not governed by permission groups. Any member of an organization with the Forms module enabled can create and print these documents.
Selecting Forms > All Forms in the left navigation opens a page-not-found error — the module's own sidebar link doesn't work. Reach MT Delivery and Empty Removal by opening /forms/mt-delivery or /forms/empty-removal directly; neither screen links to the other.
Neither form validates what you enter — every field on both forms is optional, and both can be submitted completely empty.
MT Delivery
Opening /forms/mt-delivery shows a landing card reading "Each MT Delivery slip is a printable one-off document — create a new one to generate its printout." with a New MT Delivery slip button — there's no list of past slips.
- Select New MT Delivery slip.
- Optionally choose a Vessel and set the Vessel date.
- Optionally choose a Voyage.
- Optionally enter the Discharge port, Vessel operator, and Container operator.
- Optionally search for and select a Container number.
- Submit the form.
| Field | Required | What to enter |
|---|---|---|
| Vessel | No | Pick from your organization's vessels. |
| Vessel date | No | Defaults to now if left blank. |
| Voyage | No | Pick from your organization's voyages. |
| Discharge port | No | Free text. |
| Vessel operator | No | Free text. |
| Container operator | No | Free text. |
| Container number | No | Type to search; there's no fixed list. |
On success, you see a toast reading "MT Delivery slip created successfully" and you're taken straight to the printable view — not to a list or profile, since neither exists for this form.

Current limitations
- Neither form validates its fields — both can be submitted with everything blank.
- Neither form has a list, profile, or edit screen. Once printed, a slip or permit can't be looked up again from within OCTO Ops.
- There is no in-app link to either screen; both are reached only by their direct address.
